 Template1_template_easy_readSample

Arthur is a 68-year-old retired factory worker who lives independently.

Following a minor stroke, he experiences reduced grip strength in his right hand and mild fatigue when walking long distances.

Arthur wants to continue preparing his own meals and visiting his local community centre without relying on family members for daily chores.

Identified Needs & Self-Directed Solutions: Meal Preparation: Challenge: Struggling to grip standard cutlery, open jars, or lift a full kettle safely.

Adaptations: Adaptive wide-handle cutlery, an electric jar opener, and a kettle tipper. Procurement Route: Direct purchase from a local mobility shop and online disability equipment retailers.

Mobility & Community Access: Challenge: Fatigue and mild balance issues when walking to the bus stop. Adaptations: A lightweight rollator with a built-in seat and tray, allowing him to rest during longer walks.

Procurement Route: Assessment and trial via a community Occupational Therapy (OT) service. Information Accessibility: Challenge: Finding health advice and local community schedules that are clear and simple to read during recovery. Adaptations: Summary leaflets provided in Easy Read format with clear icons, large text, and bullet points.

Procurement Route: Local self-advocacy groups and NHS Easy Read patient information hubs. Practical Exercise for Trainees & Practitioners Task: Select an everyday routine (e.g., attending a GP appointment, applying for a bus pass, or managing daily medication) and convert a standard paragraph of complex text into an Easy Read format using the three-column structure below.  Exercise Template Image / Icon Description Easy Read Text (16pt–18pt, direct language) Notes for Practitioner [Description of image, e.g., A calendar s
