How would you support someone to stay Healthy?

Some people if they feel they have had a bad experience from a health profession they may be scared to get checked whether are unwell or not.
For example if someone has had a bad experience from the dentist, how would you support and empower them to see they get check from the dentist?
The budget on benefits is rather for most people, some people with disabilities health problems can’t handle their money, and how would you support to see that they have healthy food?
Healthy food isn't that cheap in some local shops.
Some shops they are local to people have very little healthy food.
Most people with disabilities and health problems don’t drive.
Some may have to get a taxi further afield to shop, which other cost them again.
Others may be bus independent but it’s not always accessible to some people if they have a big load of shopping.
Some people with certain with disabilities need help cooking fresh food such as chopping, peeling, carrying heavy saucepans and carrying hot meals, meat out the oven for example.
There are special cooking items but for most people it costs a lot.
Some people don’t understand the information on the medications so some people are in risk of overdosing by mistake or taking less than the amount they should.
Some Gps don’t give full checkups but others don't at all.
What are the Gps missing or not even allowed to check?
If the Gps aren’t responsible who is?
There have been so many people with disabilities and health problems die unnecessary and most people have only been under the age of fifty.
It’s very important that you as a Health profession learn the way people with disabilities and health problems communication because their/ our lives are as important as yours.
I'm not saying that everyone faces these barriers but most do mainly while we have got the cuts going on.
